When you send us a request through this website we collect your name, phone number, email address and whatever you tell us about why you are getting in touch.
We use it to call you back and arrange an appointment. That is all. We do not sell it, we do not rent it, and we do not pass it to anyone outside the practice.
Health information you give us once you are a patient is covered by HIPAA and by our Notice of Privacy Practices, which you can pick up at the office or download from our new patients page.
